Paxton Forces Texas Children’s to Pay $10M, Open Detransition Clinic

Attorney General Ken Paxton settled with Texas Children’s Hospital to stop gender-transition procedures on minors and to require the hospital to open a Detransition Clinic while holding providers accountable.

Texas Attorney General Ken Paxton negotiated a deal that ends the hospital’s practice of providing gender-transition interventions to children and forces the creation of a clinic for people who want to reverse those treatments. The agreement also includes financial penalties and disciplinary steps aimed at physicians who took part in those programs.

The settlement requires Texas Children’s Hospital to pay $10 million after allegations of billing Texas Medicaid for inappropriate and illegal “gender-transition” interventions, and it mandates the hospital establish the nation’s first multidisciplinary Detransition Clinic. For the first five years, all services at that clinic will be paid for by the hospital and provided to patients free of charge, according to the terms laid out by the attorney general’s office.

Beyond funding the new clinic, the agreement forces staffing and governance changes that are meant to prevent future violations. The hospital agreed to terminate privileges and bar rehiring of several physicians tied to the disputed programs, to amend bylaws to strip privileges from anyone who violates the state’s prohibition on these interventions for minors, and to implement new compliance and ethics measures.

Attorney General Ken Paxton secured a historic settlement with Texas Children’s Hospital (“Texas Children’s”) that compels the creation of the country’s first-ever Detransition Clinic; requires the hospital to pay $10 million for billing Texas Medicaid for unallowable and illegal ‘gender-transition’ interventions, including by using false diagnosis codes; and compels the termination and revocation of privileges of multiple physicians.

After a years-long investigation by the Healthcare Program Enforcement Division, Attorney General Paxton has negotiated a historic settlement that will help protect Texans. Under the terms of this landmark agreement, Texas Children’s will establish the first-ever multidisciplinary clinic designed to provide medical care to patients who were subjected to “gender-transition” procedures. This Detransition Clinic will help patients reverse the damage caused by ideologically-motivated physicians who harmed patients by performing dangerous medical interventions for the purpose of “transitioning” them. For the first five years, all services provided through the Detransition Clinic will be funded by Texas Children’s and be free of charge to patients.

As part of the settlement, Texas Children’s has also agreed to fire, permanently and irrevocably terminate all existing privileges, and never again hire or credential five woke doctors who performed harmful medical interventions on Texans. As part of the settlement, Texas Children’s agrees not to provide “gender-transition” services; will implement a host of compliance and ethics measures; and the hospital will amend its bylaws to trigger automatic relinquishment of privileges for any physician who violates Texas’s prohibition on medical interventions to “transition” kids. Attorney General Paxton secured this historic settlement in coordination with President Trump’s Department of Justice.

Paxton praised the outcome, calling it “a monumental day in the fight to stop the radical transgender movement.”

“In addition to helping establish the first-ever Detransition Clinic and securing millions, this settlement will ensure that the deranged child mutilators who hurt our kids are fired and held accountable,” he added.

The settlement follows a probe Paxton opened in May 2023 into Texas Children’s over its provision of puberty blockers, hormones, and other treatments to minors. Investigators say the hospital continued prescribing those interventions even after state lawmakers moved to block gender-transition procedures for children.

State officials framed the agreement as enforcement of Texas law and as a move to protect young patients from inappropriate, irreversible medical steps driven by ideology rather than long-term science. The Detransition Clinic, funded by the hospital for its opening years, is positioned as a direct remedy for those harmed by prior treatments.

“I’ve been clear that any ‘gender transitioning’ procedures that hurt our children constitute child abuse under Texas law,” Paxton said, according to the Austin American-Statesman. The attorney general described the settlement as a practical example of enforcing that position and of holding institutions and clinicians responsible when state protections are ignored.
